By confirming this declaration electronically (by ticking the checkbox), I hereby declare and agree to the following terms regarding my visit to Intra lighting d.o.o., Vrtojbenska cesta 50, 5290 Šempeter pri Gorici, Slovenia, or any affiliated company (hereinafter: the “Company”).
I make this declaration on my own behalf and, where applicable, on behalf of the company/organization specified during registration, as well as the company/organization by which I am employed or for which I perform work on another basis.
I acknowledge that my electronic confirmation constitutes my legally binding consent and has the same legal effect as a handwritten signature.
1. Company Visit
The visit to the Company, organized by the Company, typically includes, but is not limited to: airport transfers, local transportation, tours of production facilities, factory and laboratories, handling of products, visit to the Lumarium Light Mastery Center, participation in lunches, dinners and tastings, participation in sports and leisure activities (collectively, the “Company Visit”). The activities of the Company Visit may vary from case to case, whereby the Company will inform the Visitor of the activities of the Company Visit in advance, or the parties will agree on such activities beforehand. For the avoidance of doubt, this statement applies to all activities forming part of the Company Visit organized by the Company
I acknowledge that activities of the Company Visit may involve certain risks.
2. Assumption of Risk
I confirm that I participate in all activities of the Company Visit voluntarily and at my own risk, and that I am aware of any risks that may be associated with or arise from activities of the Company Visit, including but not limited to: movement in production and laboratory areas, use of equipment and handling of products, transportation, consumption of food and beverages, sports and other physical activities.
3. Health Declaration
I declare that:
- I am medically and otherwise fit to participate in the above-mentioned activities of the Company Visit,
- I have no health conditions that could increase the risk to myself or others during activities of the Company Visit,
- I am obliged to inform the Company in a timely manner prior to the Company Visit of any relevant medical conditions, allergies, or limitations that may affect the Company Visit.
4. Limitation of Liability
By confirming this declaration electronically (by ticking the checkbox), I declare that the Company, its directors, representatives, owners, employees, contractors and contractual partners, affiliated persons and legal successors shall, to the extent permitted by applicable law, shall not be held liable for any injury, illness, death, loss or damage arising out of or in connection with the Company Visit, or during activities of the Company Visit or in connection therewith.I declare, for myself and on behalf of my legal successors and/or next of kin as potential beneficiaries, to the extent permitted by applicable law, that I waive any claims for damages and any other claims arising from activities or risks of the Company Visit, including but not limited to ordinary damages, loss of profits, non-pecuniary damages, any indirect or direct damages, consequential damages, reflective damages or any other form of damage, and that I shall not seek from the Company, its directors, representatives, owners, employees, contractors, contractual partners, affiliated persons and legal successors any compensation for damages arising from or in connection therewith the activities or risks of the Company Visit, including but not limited to ordinary damages, loss of profits, non-pecuniary damages, any indirect or direct damages, consequential damages, reflective damages or any other form of damage.
5. Insurance
I acknowledge that I am solely responsible for arranging and have arranged:
- my own health, accident and travel insurance,
- insurance of my personal belongings and equipment.
The Company assumes no obligation to provide insurance coverage for the Visitor.
The Company shall not be liable for my personal belongings or property.
6. Compliance with Rules
I undertake to:
- comply with all safety instructions,
- follow the Company’s internal rules,
- immediately follow the instructions of employees and responsible persons.
and, to the extent reasonably possible, act in a manner that does not increase and/or reduces the risks arising from the Company Visit.
I understand that failure to comply may result in limitation or termination of my visit.
7. Final Provisions
By confirming this declaration electronically (by ticking the checkbox), I confirm that:
- I have read and understood this declaration,
- I agree to it freely and voluntarily,
- I am aware of its legal consequences.
I acknowledge that the Company processes my personal data in connection with the Company Visit, which I provide to or disclose in this statement, in accordance with the Privacy Policy, available at: https://www.intra-lighting.com/privacy-policy.
This declaration shall enter into force on the date of electronic confirmation.
